Quality and outcomes

⭐⭐⭐⭐

Medicare Hospice CAHPS rating

93.5% of patients received a nurse or social worker visit during at least two of their final three days of life (Minnesota avg: 77.4%, national: 70.4%).

100% of patients received a comprehensive needs assessment at admission (Minnesota avg: 97.9%, national: 96.7%).

95.5% of caregivers reported being treated with kindness, dignity, and respect (Minnesota avg: 92.4%, national: 92.3%).

Hospice eligibility criteria

Cancer, advanced heart or lung disease, neurological conditions such as ALS or advanced dementia, and general functional decline can all qualify, based on clinical criteria for each.

Eligibility isn't limited to a precise timeline. Our clinical team can help assess appropriateness even when the picture isn't clear.

Enrolling in hospice means stepping back from curative treatment. If a patient wants to continue treatment, palliative care may be the better fit, and we can help make that distinction.
